Our Step-by-Step Process
We begin with a complimentary 15–30 minute phone call to understand your needs, concerns, and goals. This call helps us determine if advocacy services are the right fit and how I can best support you.
If we move forward, I’ll provide you with a Comprehensive Patient Intake Form and a Service Agreement/Consent Form. These help me gather essential medical, logistical, and personal information to serve you effectively.
After reviewing your intake form, I will identify priorities and challenges, create a customized advocacy plan with next steps, schedule any needed meetings or appointment support, and set communication preferences (calls, email, in-person visits, etc.)
We begin active services based on your needs. This may include accompanying you to medical appointments, coordinating care between doctors, managing medications or explaining treatments, resolving billing or insurance issues, or advocating during hospital stays or transitions.
Health needs evolve, and so does our plan. We will check in regularly to assess progress, adjust goals or services as needed, and provide continued resources and guidance.
